If you received a Dark Web Notification on any of the below information, here are some next steps to help protect yourself:

1. Username: Consider changing your username and the passwords linked to it.
2. Password / Password Combo List: Change your relevant passwords. For more information, refer to the following article: Learn more about Password Combo List notification.
3. Email: Consider changing your username/password linked to the email.
4. Phone number: If applicable, register your phone number on the national do-not-call registry (US only). You can also contact your service provider to block calls from suspicious numbers.
5. Insurance information: Identity thieves can use any of your insurance information to submit false claims for various categories, such as medical, car, life, home, or pet
   
   • Ensure you always review your insurance claims and bills. 
   • Look out for medical or other insurance collection notices on your credit report that you do not recognize or from your health plan saying you reached your benefits limit.
6. Gamer tag: Change your relevant passwords. Always try using a different password and gamer tag for each gaming platform.
7. Credit Card / CVV or CCV:  Contact your credit or debit card company for a replacement card and monitor your activity closely. Report any suspicious activity to your financial institution immediately.
8. Passport: Contact your local passport office to file a complaint. You can also apply for a replacement passport.
9. (New) Medical Record Number (MRN): This is a unique number found on your patient ID and can help you track your medical history (diagnosis, treatments, test results).
10. (New) Medical Beneficiary Identifier (MBI): Also known as the Medicare Number. It is a unique number found on your Medicare card (US only).
11. (New) National Provider Identifier (NPI): A 10-digit numeric identifier managed by HIPAA for healthcare providers.  (US only).
12. (New) National Health Services Number (NHS): A 10-digit number updated in your health record. You can find it on a prescription, test results, or an appointment letter (UK only).

General tips:

• Remove mail from your mailbox right away. You can also request a mail hold.
• Consider setting up a locked mailbox to decrease the risk of stolen mail.
• Shred anything that has personal information.
• Opt for paperless delivery whenever possible and practical.

1. Company name: Notify your internal legal, IT, or compliance teams. You can also report any fraudulent activity to your local business registry.
2. Creditsafe number: Notify Creditsafe, local credit bureaus, or trade authorities of any fraudulent activity.
3. DUNS number: Stay alert for fraudulent use in supply chain, vendor, or customer relationships. You can also notify Dun & Bradstreet (D&B), credit bureaus, or trade authorities.
4. Employer ID number: Check for fraudulent tax filings, employment records, or HR systems for unauthorized access. Notify local tax authorities, agencies, or the Internal Revenue Service. You can also place a fraud alert or credit freeze where applicable. 
5. Company SOS number: Look for unauthorized legal filings and notify the Secretary of State or the respective state business registries if there’s potential misuse. You can also review your business registration or licensing records regularly.
6. Taxpayer ID number: Check for fraudulent tax filings, employment records, or HR systems for unauthorized access. Notify local tax authorities, agencies, or revenue services. You can also place a fraud alert or credit freeze where applicable.
7. Domain name: Consult with your local domain registrars or cybersecurity providers for regional best practices. Update your domain registrar credentials. Renew, or transfer the domain if necessary.
8. VAT number: Review and monitor VAT transactions and filings for any irregularities. You can also notify your local VAT or sales tax authority of any fraudulent activity.
